Indtil videre er de tilg\u00e6ngelige behandlinger baseret p\u00e5 b\u00e5de kirurgiske og ikke-kirurgiske tilgange. Det kan v\u00e6re alt fra kost\u00e6ndringer til tarmtr\u00e6ning eller stimulering af sakralnerven, men ingen af dem giver en langsigtet l\u00f8sning. Nye behandlinger baseret p\u00e5 regenerativ medicin er p\u00e5 vej frem, og de har til form\u00e5l at regenerere lukkemusklen og genoprette kontinensen. Normalt best\u00e5r disse af indgivelse af en suspension af ekspanderede skeletmuskelceller (SkMDC'er) til det beskadigede sted. Men denne strategi resulterer ofte i en reduceret cellelevedygtighed p\u00e5 grund af behovet for at h\u00f8ste celler fra ekspansionsplatformen samt den ikke-naturlige brug af en cellesuspension til at levere de forankringsafh\u00e6ngige celler. I denne unders\u00f8gelse foresl\u00e5r vi proof-of-concept for bioprocessing af en ny celleleveringsmetode til behandling af f\u00e6kal inkontinens, opn\u00e5et ved en skalerbar totrinsproces. F\u00f8rst blev patientisolerede SkMDC'er ekspanderet ved hj\u00e6lp af plane statiske dyrkningssystemer. For det andet blev de ekspanderede SkMDC'er kombineret med biokompatible og biologisk nedbrydelige (dvs. direkte implanterbare) poly(m\u00e6lkesyre-co-glycolsyre)-mikrob\u00e6rere fremstillet ved termisk induceret faseadskillelse ved hj\u00e6lp af en PBS-MINI Vertical-Wheel\u00ae-bioreaktor til engangsbrug. Denne proces gjorde det muligt for op til 80% effektivitet af SkMDC'er at s\u00e6tte sig fast p\u00e5 mikrob\u00e6rerne. Det er vigtigt at bem\u00e6rke, at SkMDC'erne var levedygtige under hele processen og bevarede deres myogene egenskaber (f.eks. udtryk for CD56-mark\u00f8ren) efter vedh\u00e6ftning og dyrkning p\u00e5 mikrob\u00e6rerne. N\u00e5r SkMDC-holdige mikrob\u00e6rere blev placeret p\u00e5 en kultursk\u00e5l, var cellerne i stand til at migrere fra mikrob\u00e6rerne til kulturoverfladen og differentiere sig til flerkernede myotuber, hvilket fremh\u00e6ver deres potentiale til at regenerere den beskadigede lukkemuskel efter indgift i patienten. So far, available treatments are based on both surgical and nonsurgical approaches. These can range from changes in diet, to bowel training, or sacral nerve stimulation, but none of which provides a long-term solution. New regenerative medicine-based therapies are emerging, which aim at regenerating the sphincter muscle and restoring continence. Usually, these consist of the administration of a suspension of expanded skeletal-derived muscle cells (SkMDCs) to the damaged site. However, this strategy often results in a reduced cell viability due to the need for cell harvesting from the expansion platform, as well as the non-native use of a cell suspension to deliver the anchorage-dependent cells.
